We use your health information to provide, coordinate, and manage your mental health and substance use treatment. This includes sharing information with other healthcare providers involved in your care — such as your primary care physician, psychiatrist, or therapist — to ensure coordinated, whole-person treatment. Example: Your therapist may consult with our psychiatric provider about your medication needs.
Payment
We use and disclose your health information to bill and receive payment for your treatment. This may include sharing information with your health insurance plan, claims processing services, and billing agencies. Appointment Reminders & Communications
We may contact you to remind you about appointments, follow up on missed visits, or share information about treatment alternatives and health-related benefits. We may contact you by phone, voicemail, email, or SMS message, unless you request otherwise.
Other Permitted Disclosures
We may also use or disclose your PHI without your authorization for specific public-interest purposes: public health and safety (reporting abuse, neglect, or serious threats of harm), health oversight activities (audits, investigations, licensure), law enforcement (court orders, subpoenas, warrants), coroners and medical examiners, organ donation, and workers' compensation programs. We may also disclose information as required by California state law, including mandatory reporting of suspected abuse or neglect.
