Intake, Treatment Planning and Care Coordination: The Foundation of Effective Mental Health Care

The quality of your mental health care is only as good as the foundation beneath it. Our Intake, Treatment Planning & Care Coordination program is designed to establish that foundation with intention, rigor, and collaboration. The intake process is a comprehensive clinical evaluation — typically completed over one to two 60–90 minute sessions — that examines your presenting concerns, psychiatric and medical history, social context, strengths, and goals. This is not a rushed screening; it is a deliberate diagnostic process.

Following the intake, treatment planning begins. At Ineza Outpatient Services, we develop your plan collaboratively — never in isolation. Research consistently shows that person-centered treatment planning improves engagement, adherence, and outcomes. Your plan will identify measurable goals, specify the interventions we will use, and outline the roles of each member of your care team. Care coordination then ensures that every part of your treatment — psychiatric medication management, individual therapy, group work, and any external referrals — operates as a seamless whole rather than disconnected pieces. Led by Caroline Ndifon, PMHNP-BC, we coordinate with PCPs, specialists, and community resources so you never have to navigate the system alone.

Understanding Intake & Care Coordination

Understanding Intake, Treatment Planning and Care Coordination

Comprehensive Intake Assessment

Your intake is a thorough clinical evaluation covering presenting concerns, psychiatric and medical history, family history, social context, substance use, strengths, and goals. We use validated tools like the PHQ-9 and GAD-7 to establish a baseline, and we complete risk assessment to ensure safety and appropriateness for outpatient care.

Collaborative Treatment Planning

Your treatment plan is developed with you, not for you. We identify measurable, person-centered goals, select evidence-based interventions, set target dates, and schedule regular reviews. The plan clearly documents diagnosis, goals, objectives, interventions, and the roles of each team member.

Active Care Coordination

Care coordination is the connective tissue of your treatment. We communicate with your PCP, specialists, and community resources, manage referrals, and ensure that every provider involved in your care shares the same understanding of your needs and goals. You never have to piece together your own care.

Navigating Intake & Care Coordination: A Collaborative Path to Recovery

Intake, treatment planning, and care coordination are not administrative formalities — they are clinical interventions in their own right. A thorough intake establishes the diagnostic foundation for everything that follows. A well-crafted treatment plan provides direction, accountability, and a way to measure progress. And effective care coordination ensures that the various parts of your care — psychiatric, therapeutic, medical, and social — function as a cohesive whole.

Care Coordination That Extends Beyond Our Walls

Research consistently shows that care coordination improves access to services, reduces psychiatric hospitalizations, decreases emergency room visits, and enhances quality of life for individuals with mental health challenges. At Ineza Outpatient Services, our care coordinators work as the central point of contact for your treatment team. They communicate with your PCP, facilitate referrals to specialists, help you access community resources, and ensure that everyone involved in your care has the information they need to make informed decisions. When appropriate, we invite family members and other supports to participate in care planning meetings.

The intake is a comprehensive clinical evaluation that typically lasts 60–90 minutes. Your clinician will review your presenting concerns, psychiatric and medical history, family history, social context, substance use patterns, strengths, and treatment goals. We also administer validated assessment tools like the PHQ-9 and GAD-7 to establish a baseline. A risk assessment is completed to ensure safety and appropriateness for outpatient care.
Most intakes are completed in one session lasting 60–90 minutes. In some cases, depending on the complexity of your history, we may recommend a second session to ensure we have a complete picture. Prior to your intake, we may send you digital forms and screening questionnaires to complete at home to save time.
Your treatment plan is developed collaboratively between you and your clinical team. We believe that the best plans are co-created, not prescribed. Your plan will include measurable goals, evidence-based interventions, target dates, and scheduled review dates. It clearly documents the roles of each team member and is updated as your needs evolve.
Care coordination is the deliberate organization of your care activities and information sharing among all providers involved in your treatment. It matters because fragmented care leads to gaps, duplication, and poor outcomes. Research shows that care coordination improves access to services, reduces psychiatric hospitalizations, and enhances quality of life. We serve as the central point of contact for your entire care team.
Yes. Our telehealth intake and care coordination services are available to adults physically located in California at the time of service. We conduct secure, HIPAA-compliant video sessions for intake assessments, treatment planning meetings, and ongoing care coordination. California law requires insurers to cover telehealth services at parity with in-person care.
With your consent, we communicate directly with your primary care provider, specialists, and any other treatment providers you are working with. We share relevant clinical information, coordinate medication management, facilitate referrals, and ensure that everyone involved in your care has a shared understanding of your treatment plan. We also help you navigate community resources such as housing, employment support, and peer programs.
We review treatment plans regularly — typically every 90 days or more frequently if clinically indicated. Reviews are conducted collaboratively with you and may involve input from your full care team. We use validated measures to track progress and adjust the plan as needed to ensure it remains effective and responsive to your evolving needs.
